How we use these details

Greater Health collects these details, including health information, to assess the referral, contact the person or their contact person, plan safe sessions and claim funding. We only share them with people involved in their care or funding, such as their GP or referrer, their plan manager or the NDIA, their Support at Home provider, Medicare, DVA, TAC or WorkSafe. If key details are missing, we may not be able to accept the referral.
